Exercise-related compensation

In one post-surgical case, a patient developed neck pain during a side-lying shoulder abduction exercise. Assessment revealed excessive cervical muscle engagement during arm elevation, particularly as resistance increased.

Instead of isolating the shoulder musculature, the neck was compensating—resulting in symptom onset.

Clinical approach to correction

Effective intervention requires more than strengthening alone. Treatment focused on:

  • Reducing cervical muscle tone
  • Providing tactile and verbal cueing
  • Reinforcing proper scapular and shoulder activation
  • Gradual progression of load with correct mechanics

Once cervical compensation was minimized, the patient’s neck symptoms improved and shoulder rehabilitation progressed appropriately.
